SYLVANIA Dream Vacation Sweepstakes

1) Sweepstakes Details: MANY WILL ENTER, FEW WILL WIN. NO PURCHASE NECESSARY. A PURCHASE WILL NOT INCREASE YOUR CHANCE OF WINNING. Sweepstakes is open only to legal residents of the 50 United States, including the District of Columbia, who are 13 years of age or older as of October 1, 2008. Employees of OSRAM SYLVANIA Products Inc. (“Sponsor”), KS Promotions, Inc. (the “Judges”), Disneyland ® Resort or any of their affiliate companies, agencies, promotion suppliers, printers and their immediate families,(parents, children, siblings and spouses, regardless of where they live) and those in the household, whether related or not, are not eligible to enter. Void in Puerto Rico and where prohibited by law. By entering, you agree to these Official Rules and the decisions of Sponsor and the Judges, which shall be final and binding in all matters relating to this Sweepstakes. Sweepstakes begins at 12:01 a.m. CT on October 1, 2008 and ends at 11:59 p.m. CT on December 31, 2008 (the “Sweepstakes Period”).

2) How to Enter:

a. Website (Alternative method of Entry): Enter by visiting www.sylvania.com/dreamvacation during the Sweepstakes Period, click on the grocery store chain where you shop and follow the directions to complete and submit an Official Entry form; OR

b. SuperValu Loyalty Card: Purchase any Sylvania branded CFL at any Acme grocery store, Shaw’s grocery store, Jewel grocery store or Albertson’s Southern California or Albertson’s Intermountain grocery stores during the promotional period and receive an entry into the sweepstakes for each package purchased. For a list of stores, please log on to www.Supervalu.com.

3) Limit: 50 entries per person regardless of method of entry Sweepstakes entries not properly completed will not be allowed. All entries become the exclusive property of Sponsor and none will be returned.

4) Laws and Regulations: All federal, state and local laws and regulations apply.

5) Prizes:

a. Five (5) Grand Prizes (one Grand Prize per grocery chain listed in section 2b) : Each winner will receive a trip for winner and up to three (3) companions to Disneyland® Resort in Anaheim, CA. Trip includes 4 day/3 night accommodations at Disneyland® Hotel (double occupancy), round-trip coach air transportation for winner and companions from the major commercial airport nearest winner’s residence to nearest major commercial airport to Disneyland® Resort in Anaheim, CA, ground transportation between Disneyland® Hotel and LAX, and Disneyland Resort Passes. (ARV: US$5,500 each). Grand Prize winners and travel companions must travel on the same itinerary, as set by Sponsor in its sole discretion. Meals, gratuities, service charges or taxes, ground transportation not specified, hotel room service, laundry service, alcoholic beverages, merchandise, souvenirs, local and long distance telephone calls and all other unspecified incidental expenses for this prize are the sole responsibility of winner. Total approximate retail value of all prizes is: US $33,000.

b. Conditions on Prizes - Unless otherwise specified herein, all taxes are the sole responsibility of the winners. Winners of prizes will be issued an IRS Tax Form 1099 stating the value of the prize for 2009 tax filing purposes. Prizes are not transferable or redeemable for cash and prize substitutions will not be allowed, except at the sole discretion of Sponsor due to prize unavailability for any reason and only then for prize of equal or greater value. Without limiting the foregoing, Grand Prize is subject to availability/blackout dates and some additional restrictions may apply. Grand Prize trip must be completed within one year from the date of prize acceptance and winner must give at least 30 days advance notice prior to intended date of departure. Sponsor, Disneyland Resort, the Judges, and their parents, affiliates, subsidiaries, officers, directors, employees, agents and representatives (collectively the “Sweepstakes Entities”) disclaim any and all representations and warranties, express or implied, in fact or in law, in connection with all prizes. By claiming a prize, winner agrees to assume all risks and agrees that the prize is awarded upon the condition that each of the Sweepstakes Entities will have NO liability whatsoever for any injuries, losses, death or other damages of any kind caused by the prize or resulting from acceptance, possession, or use/misuse of the prize or from winner’s participation in the Sweepstakes.

6) Drawing and Notification Details: On or about January 31, 2009, five random Grand Prize drawings will be conducted from all eligible entries from each grocery store chain listed in section 2b – one drawing from Acme locations, one drawing from Shaw’s locations, one drawing from Jewel locations, one drawing from Albertsons So. CA locations and one drawing from Albertson’s Intermountain locations. The drawings will be conducted by the Judges at their offices in Carlsbad, CA. The odds of winning will be determined by the total number of eligible entries received for each grocery store chain. Winners will be notified by mail on or about February 15, 2009. Grand Prize winners and companions (if companion is a minor, his/her parent or legal guardian) will be required to sign and return to Sponsor an affidavit of eligibility and publicity/liability release within 14 days following the date of notification of Grand Prize winning. Winners will be disqualified and prizes will be awarded to alternate winners (selected from all remaining eligible entries) if all affidavits/releases are not signed and returned as required, or if prize notification or prize is returned as undeliverable. By accepting a prize, winners agree to the use of their name and/or likeness in any promotion/advertising/trade activities, and any and all rights to said use, without further compensation or notification, and without prior review and approval, except where prohibited by law. Limit of one prize per household.

7) General:

a. Sponsor is not responsible for lost, late, damaged, postage due or misdirected mail/entries. In addition, Sponsor is not responsible for any incorrect or inaccurate information, whether caused by Website users, by any of the equipment or programming associated with or used in the Sweepstakes, or by any technical or human error that may occur in the processing of entries in the Sweepstakes.

b. Sponsor is not responsible for any problems or technical malfunction of; computer online systems, servers, or providers; computer equipment; computer software; or failure of e-mail or entry on account of technical problems or traffic congestion on the Internet or at any Web site or combination thereof, including injury or damage to participant’s or to any person's computer related to or resulting from participating or downloading materials from the Web Site or Sweepstakes. If, for any reason, the Sweepstakes is not capable of running as planned, due to without limitation, infection by computer virus, worm or bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Sponsor, that in the sole opinion of Sponsor, could corrupt or affect the administration, security, fairness, integrity, or proper conduct of this Sweepstakes, Sponsor reserves the right, at its sole discretion, to cancel, terminate, modify, or suspend the Sweepstakes. In such event, winners will be selected from among all eligible mail in entries and all eligible online entries received prior to and/or after (as appropriate) the action taken by Sponsor or by some other method determined by Sponsor in its sole discretion as fair, consistent, and in accordance with the spirit of these Official Rules. And, notice of such action by Sponsor will be posted on the website. E-mail entry will be deemed made by the authorized account holder of the e-mail address submitted at the time of entry. The authorized account holder is the natural person who is assigned to the e-mail address by an Internet access provider, online service provider, or other organization that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Entries may not be reproduced in any manner and entries that are lost, misdirected, delayed, incomplete, illegible, defaced, mutilated, forged or in any manner altered will be disqualified.

c. Information Collection Practices. Personal information collected in conjunction with this Sweepstakes will be used by Sponsor: (1) to administer the Sweepstakes, including notification of winners and inclusion of winners’ names on a publicly available winners list and (2) for future advertising/marketing efforts of Sponsor. Sponsor will share personal information with the Judges in connection with their duties in the Sweepstakes.
